The new DOT drug testing chain of custody forms (CCF) are required to be used as of June 30, 2018; use of the old form will cause significant delays in your drug testing results.

How do I know if I have the old or the new DOT drug testing chain or custody forms (CCF)?

To determine if you have the correct version of the NEW CCF Form, review the forms you have for the following changes:

  • In Step 1D: Removal of the checkbox, the letters “DOT” and hash line in front of the text “Specify DOT Agency”
